What services are not covered by OHIP?Examples include but are not limited to: Drivers physicals, employment physicals, children’s camp physicals, newborn circumcision, removal of some moles/ cysts, completion of forms, sick notes, medical certificates, medical-legal correspondence 

How do I pay for services not covered by OHIP?The Business Office will accept your payment via cash, cheque or debit. At this time we do not accept credit cards.

I've dropped off my paperwork for my doctor to complete.  How long can I expect for paperwork/insurance forms to be completed?The completion of paperwork varies per physician. Paperwork can take anywhere from a few days to several weeks to be completed.  A staff member from the Business Centre will call you when your forms have been completed.

How do I renew my medications?Call your pharmacy and they will fax our office a request. Please ensure to give at least 2 weeks’ notice for medication renewals

Can I walk in for my blood work, or do I need to book an appointment?Appointments are required although they may accept walk-ins the day of. To book an appointment visit Life Labs' website or call 1-877-849-3637 X 4.

What are Life Labs' hours?Monday-Friday 7-12 & 1-3.  Closed first Wednesday of every month at 12 p.m.  Please refer to Life Labs' website for their most updated hours.

How long are blood work requisitions "good" for?Lab requisitions are valid for 6 months. For non-urgent blood work, you should wait 1-2 days after the requisition has been faxed to the lab prior to getting your blood work done.

Why can't I get through to your clinic?We continue to experience a large volume of calls, please be patient.

What is the process for a referral to a specialist?Referrals get sent to a Specialist office from your Family Physician.  Once received at the Specialist’s office the referral will be triaged according to urgency.  Wait time varies from office to office.  When the referral is triaged either the Specialist office or the Family Physician's office will contact the patient. 

Are you accepting new patients?We do keep a waitlist! Currently the list is very large (approximately 2000+ people). If you’re interested in being added to the list, please complete our Request for Physician form.

I'm currently a patient of the Listowel Clinic and have found a new doctor.  How does my new doctor get my records from your office?At your new physician’s office they will have you sign a release of records transfer form. They will then fax your consent to our office. We will prepare your records and fax or mail them to your new doctor’s office.

I've been accepted as a new patient at the Listowel Clinic.  How do I transfer my records to your office?Visit our Business Centre at the clinic. The staff will have you sign a release for transfer of records form from your previous doctor and fax the request to their office. There may be a fee for this service from your previous doctor’s office that is not covered by OHIP. Please have the correct spelling of your physician's name before you come. 

Can my naturopath or chiropractor get a copy of my test results? Is there a charge?You may request copies of your results and pick them up in our Business Centre to take to your Naturopath or Chiropractor. There may be a fee.
